Information Needed to Put Employees on Payroll

There are payroll forms you must have an employee fill out with their personal information prior to putting them on payroll. 


These forms include:

  • Current W-4
  • Form I-9 (Employment Eligibility Verification)
  • Certificate of Residency
  • Direct Deposit Authorization (if applicable)


You also will need copies of acceptable documents to establish identity and employment authorization; these documents are listed on page 2 of Form I-9. Some acceptable documents are a U.S. Passport, Driver’s License, and Social Security Card. You must retain these documents for any current employee as well as for former employees for at least 3 years after employment is terminated.
